There's code for her in a few spots on disc 2, notably Great Glacier and the Fort Condor huge materia battle. Cloud was originally going to save her at the end of disc 1 and then she'd be killed off at some point on disc 2 (probably in the Crater when Sephiroth was reawakened), but they decided to move it up in the game.

Saddest scene in any game I've ever played, bar none :[ This is sadder then Dom dying in Gears 3 or any of that.
