Agreed. There's actually grotesque stuff on gelbooru. And the patient's wearing an oxygen mask. So there's really nothing grotesque about it. People running gelbooru should look up the "guro" images here and see what I'm talking about.

My guess is it's problaby the first sentinel if you don't know it's mearly a story arc in the FrankenFran universe which homages the first four kamen rider like sentinel (Ichigou) sentienal 2(Nigo) vengence 3 (V3) and finally sentienal man (rider man, V3's partner and the fourth rider)
