Well, the sign says "Anything goes, just cut off some extremities." Right beneath that, but still on the sign, you can see a woman holding a head of a decapitated kneeling male (monster?) So I guess it means to have sex, it's neccessary to get rid of the head (first?)
The poison's glow seems to indicate the state of a monster. pink means dead (because the head's gone), blue means alive (still fucking (motion blur)).
Okay, so the head goes after the female is done. But although the woman on the left seems to be done with that monster, she hasn't had enough yet. She is probably aiming for the same state the other customers are in. Obviously because the servant is rushing over to her, with a stamina (scorpion) drink (alcohol), a new poison cartridge.

Anon above me, you fail at so many levels. First, the artist is asian, not russian. Second, the things that lose their heads, aren't humans. This is obviously some twisted mmorpg world, where killing monsters comes naturally. Third, although even russians at times find it hard to understand the fuck is going on in the mess we call our country. I still see no connection between this asian hentai thing and you not understanding russians. Fourth, there is no doubt in my mind that you are a retard and understand nothing at all.
