I tried reading the story behind this but I'm not fluent in Japanese. If I read it right then Yuuko was mortally wounded in an attack by Nani. Here, Nano is trying to save her life with a combination of mouth-to-mouth and Nano-machines (no pun intended) introduced into her guts through her mouth. The only way the Nano-machines can save her life is by making her into a cyborg of sorts. [Note the key forming on Yuuko's back.]
