The title of Sanin was given out as a joke by Hanzo. He could have easily destroyed the three of them, but he felt that branding them with the name 'legendary losers' would be a far better fate. Over the years it mutated into some warped form of respect for the three.

Hanzo was fucking notorious for giving strong people nicknames. He could have definitely beat them but he did "not" brand them the "Legendary Losers", he did let them live though.
"San" means three and "nin" is a contraction for ninja so Sannin means "Three Ninja". That means he gave them the nickname "Legendary Three Ninja". . .not "Legendary Losers".
