MILF has more to do with age and body than with actual motherhood. (Though I'd imagine a young woman with a young body type could still potentially qualify if she's a mother now)

This goes for both tagging purposes, pornography in general, and probably even people on Earth.

Though something like "cougars" is a kinder term for non-pornographic examples, and could be used for something who never had children, Gelbooru doesn't use this. And most porn sites don't either.
