Why does it freak you out? It's just muscle definition and her kneecap. In my opinion, this is a really great picture! I love the shading and the artist did a great job at making each material look realistic (like rubber, leather, etc.).

The left leg is attempting to be accurate but it throws off the beauty of this picture because the kneecap was poorly done.

I feel like the image as a whole is slightly over-rendered. The knee is sort of weirding me out on its own merits.
Her right arm is also somewhat disturbing. I don't think the bones of your elbow are supposed to look like that.
