I like tentacles, but not all the way through. Either its going all the way through their colon, intestines, stomach and then out through their mouth (gross), or tearing a hole in the top of the uterus, going through your chest cavity, then tearing a hole in the esophagus (ouch)

Let's call that tentacle what, a 2 inch diameter? The small intestine alone is about 22-25 feet, so a tentacle going the length of the digestive tract would be what, about 2/3 of a cubic foot in volume?
Of course the intestine itself would be stretching to about double its usual capacity... at the least is would be excruciatingly painful, and I doubt it would be survivable in the long term without some sort of magic (healing, shapeshifting, stretching powers, whatever).
On the other hand, more realistic than about 2/3 of the stuff in hentai.

All the way through is usually done by simply going through the female body's natural exit. Having an average sized penis fit in the large intestine is easy, as it can stretch far enough. The problem is intestines aren't meant to be constantly full at all times.
