That face is actually meant to be read as intense pleasure.

At least in porn Japanese take it to extremes of hysteria that don't seem appropriate for Westerners, who prefer a more moderated expression with 'satisfied' moans.

AFAIK the whining that often accompanies this isn't usually to do with unpleasant distress but to reinforce (absurdly for my tastes) the emotionally overwhelming pleasure the woman is experiencing.
