As far as i know, Gehrman created the doll in the old "real" workshop after the death of Lady Maria, a hunter with who he probably fell in love ( could be many other things too tho, but the description of her clothes and the item you pick in the old workshop makes it clear that he did everything to make the doll beautifull ).

At that time, the hunter nightmare ( the thingy that let your character "revive" ) wasn't created yet and it's probably the death of Lady Maria that trigered Gehrman's link to the blood moon :
In the "dream" workshop, the doll is alive unlike the real workshop that Gehrman left, where the doll is simply an object.

So, Anon1, Maria doesn't love the hunter but maybe Gehrman, and she isn't the doll either. It's just that the first hunters got cursed by Croz, and even after their death they're wandering within the "old hunter nightmare".
She must have been here for so long that she lost her mind, killing everyone that wander in the nightmare over and over.

And Anon3, i think this kiss is just like the "death kiss", meaning she will kill you soon enough.

That instant riposte is such hot bullshit. I still can't comprehend how people find her fight easy or enjoyably challenging. Plus that "NUTHIN' PERSONNEL"-tier edgelord fighting style of her's just makes me loathe every encounter.

I must be in this weird minority that absolutely despises her, but enjoys the Orphan of Kos fight. That's a rough fight, but the absurd reach and power he has at least feels more justified and fair.

Actually if you look at the vids showing deleted content, Maria had dialogue and would speak with fondness with our character. It showed she was aware on some level of her and the Doll's "connection" and also that you were part of the weirdest love.. bucket?
