This is a sequel image to #3316982. She's been fed oilfish, which causes nasty diarrhea when you eat enough of it because the oil in the fish isn't totally digestible.

Here, she's being offered a slice of fatty tuna in celebration of having recovered from said nasty diarrhea, but she thinks it's still oilfish on account of them looking similar and because her hands have been bound. Hence the reaction.
