How does the rating system come to play on violence but without sexual content?

Violence is not taken into account. If it's gruesome use the guro tag. Rating is for sexual context only.

And now that I think about it shouldn't scary and horror images be put into questionable on the basis that it could scare minors (not users obvi, well maybe that too)?

It is a violation of our ToS for minors to use the site, so we do not base any policies around protecting them other than banning them as soon as we find them and reporting anyone who claims to have abused or have intent to abuse them to the National Center for Missing and Exploited Children.
